Scheduling Chaos

Pine Park Health faced a critical challenge: efficiently scheduling primary care providers across multiple senior living communities. Providers were juggling packed schedules filled with patient visits and administrative tasks, leaving little room for flexibility. In a value-based care model, where high provider utilization drives revenue and improves patient outcomes, every minute matters—but inefficiencies leached their time.

Some seniors enjoyed flexible appointments (think homebodies), while others needed fixed times to accommodate busier schedules or include family members. The existing system treated everyone the same.

Scheduling was a tedious, error-prone process for providers and care coordinators, leading to awkward gaps and stressful days.

No Room for Urgency

Without reserved time, providers struggled to address sudden health issues or complete visit notes, risking burnout, care delays, and patient dissatisfaction with appointment reschedulings.

Custom Tailored Scheduling

Pine Park Health partnered with Redefined to build a custom web application that transformed their scheduling process. The solution tackled every pain point with precision:

Single Source of Truth

The provider calendar moved from the Electronic Health Record (EHR) system to a user-friendly internal web app, syncing non-clinical appointments from Google Calendar to avoid conflicts.

Patient-Centric Scheduling

A Calendly-inspired interface enabled providers and coordinators to book appointments effortlessly, automatically respecting patient preferences for fixed or flexible times. A “best time” algorithm minimized travel by grouping appointments in nearby rooms or wings, packing schedules efficiently to leave larger blocks of time available for critical clinical tasks.

Built-In Flexibility

The system reserved “Flex Time” for providers to handle urgent needs and wrap up notes, while preventing overbooking to maintain balance.

Seamless Access

A Google Chrome extension overlaid the app on the EHR, keeping everything in one place. The calendar included patient details and quick links to start visit notes or join telehealth calls.

Reliable Syncing

Real-time updates from Google Calendar and EHR webhooks kept data accurate. Routine full syncs filled any gaps, ensuring nothing slipped through the cracks.

A Peek Behind the Curtain

We built the solution with a modern tech stack, starting with a responsive and intuitive frontend crafted using Remix.run, React, and Vite, ensuring a seamless user experience for providers and coordinators alike. Behind the scenes, the application relied on PostgresSQL for robust data storage, Redis for caching, and BullMQ for efficient job queuing, ensuring reliability and performance even under heavy use. Seamless integrations with Elation Health and Google Calendar streamlined operations, synchronizing the scheduling system across the company’s tools. Hosted on Google Cloud Platform and Google Cloud Run, the system was designed for scalability and resilience, capable of handling the demands of a growing senior care network. To ensure smooth, repeatable, and reliable deployments, the team leveraged Terraform CDK for infrastructure as code and GitHub Actions for continuous integration and delivery. This powerful combination enabled rapid iteration, with updates released multiple times per day, allowing the solution to evolve quickly in response to user feedback and changing needs. These technologies worked together to create a system that was as dependable as it was user-friendly.
